• Provide friendly visits to involve social or religious activities
• Light to heavy housekeeping and laundry
• Meal planning and preparation
• Delivering groceries and supplies
• Grounds maintenance
• Pet care
• Registered Nursing oversight

Home Health Aides

• Home Assistant services (see above)
• Maintain calendar of events and appointments
• Escorted transportation (mileage is not charged)
• Food intake monitoring
• Medication reminders
• Registered Nursing oversight

Certified Nursing Assistants:

• Assistant/Home Health Aide services listed above
• Bathing and showering
• Dressing and grooming
• Assistance with eating
• Toileting and incontinence care
• Mobility with ambulation
• Over-night respite care
• Registered Nursing oversight

Registered Nursing Care

• Complete initial assessment and develop an individualized Care Plan with clients and family, in collaboration with the physician
• Ongoing supervision of all care and services
• Education to clients and their loved ones
• Provide skilled care for ones complex medical needs such as, but not limited to: IV therapy, wound care, ostomy care, tracheotomy care, etc.

Social Services:

• Provide Case Management and support for each client in conjunction with The Care Team, family and physician
• Identify available community resources for client and family
